1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

1. Maintaining physical and mental wellbeing by gentle exercise classes and outings.

2. Encouraging Social inclusion for people dignosed with Dementia and their carers.

3. Providing advice and signposting for Legal and Financial matters.

4. Support for bereaved members by companionship and inclusion.

5. Liaison and signposting to other charitable organisations.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

1. Obtaining year on year financial support is always a challenge

2. Recruitment of active volunteers is a challenge.

2. Actions

  • To offer gentle exercise classes and Outings for over 40's.

    We offer these classes to help maintain physical and mental health in our clients which includes people living with a Dementia diagnosis and their carers.

    We offer advice and support on all financial and legal matters, signposting to other charitable organisations as appropriate.

  • To offer companionship and inclusion for people living with Dementia.

    We offer support and companionship to people in the Grantham area. We offer support to bereaved members. We have volunteers to help us but we continue to struggle to find enough volunteers and enough funding to continue year on year.
