1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

As a team we are increasingly aware of the prevalance of dementia within the community.  As a business we would like to support people with dementia in any way that we can.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We will review the effectiveness of our action plan at our practice meetings every 6 weeks.

  • Dementia Friends

    Arrange 2 Dementia Friends sessions within a lunch and learn session to cover all staff

  • Dementia Friends Champion

    Arrange one member of the MCVH team to attend a Dementia Friends Chamion Induction day and become a champion.  They will then be able to deliver the information session to any new starters.
