1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We believe that the Practice has a key role to play in delivering better outcomes for people with dementia and their carers; the Practice has undertaken considerable staff training with many becoming dementia friends.  In addition, the Practice has worked with patients with dementia to ensure that the premises meet their requirements.  This resulted in revised signage, improved lighting, strategically placed chairs for sitting, and removal of mats.  The Practice is a member of the Safer Places scheme. 


The Practice is keen to enable patients to access dementia assessments, and to refer on for diagnosis and appropriate tests.  The Practice works closely with an Admiral nurse who provides one-to-one support for patients and carers.  All staff members have access to information about local dementia services and help sheets which patients can be signposted to, this information is also available on the website and kiosk available in the waiting room.  In addition, the Practice has a number of social prescribing services that it runs from the site including coffee mornings, gardening groups, singing for health, chair exercises, cancer care support group, walking groups.   The Practice participates in all enhanced services available to identify and support patients with Dementia and their carers. 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Patients and carers are often fearful about receiving a diagnosis of Dementia and can avoid assessments.  The Practice needs to break down such barriers, and provide reassurance and appropriate signposting / support for patients and carers who do receive a diagnosis.  It is important that patients can access timely diagnosis, where appropriate, as it can enable people to access more support that is not available to them otherwise. 

The prevalence of dementia is set to increase considerably, the Practice needs to ensure that its services meet the diverse needs of the population with this disease, and that the building enables people to access the services they need.

The Practice is committed to continuing staff training to achieve these goals.

2. Actions

  • Signage

    The Practice has consulted patients with dementia, and organisations who have expertise, to revise and improve the signage in the Practice.  This resulted in increased signage in high contrast colours, including pictures where appropriate, and doors to help patients find the way out of bathrooms etc.  Mats have been removed.  In future, when furniture is replaced we will look for high contrast furniture.  A quiet room is available for patients to wait if they are struggling in the waiting room, a member of staff will notify them when and where to go to their appointment.

  • Staff Training

    The staff team has undertaken dementia friends training provided by Wakefield Council and most became dementia friends.  Additional training has also been provided in the form of video and radio reports and online training.  The Practice is committed to providing ongoing staff training in relation to dementia.

  • Signposting

    The Practice has developed systems and processes to enable all members of staff to signpost patients, and carers, to a wide range of dementia services provided by both statutory and voluntary sector organisations, a record is kept of such signposts and information can be printed.  The Practice works closely with an Admiral Nurse and the Health and Wellbeing Development team who provide support to patients and families.  In addition, the Practice has put a wide range of dementia resources on the website www.churchstreetsurgery-ossett.co.uk.  The Practice also works closely with Carers Wakefield to provide support to carers.  The Practice works closely with health champions to identify patients who are carers.  When new patients register we offer them access to information / services depending on their needs and ask all patients if they have a carer, or are a carer.
