Park Court is a Yorkshire Housing scheme located in a prime position of Thurnscoe village . Park Court offers independence and wonderful amenities . The scheme includes 45 one and two bed roomed bungalows with access to communal longue ,laundry ,garden and guest facilities

1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
Park Court will be aware of anyone with dementia who becomes a tenant .we will provide support ,information and signposting where appropriate.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
Encouraging understanding and tolerance amongst other tenants.
Keeping up to date with signs of changes in tenants cognitive performance and addressing concerns without causing offence or distress

Dementia Awareness and Dementia Friends
As the Independent Living officer for Park Court I will sign up as a dementia friend on line and I will encourage other people to do so - I will also attend a Dementia Friends Session to further enhance may dementia knowledge of dementia. I will wear my Dementia friends badge to raise awareness of the Dementia Friends initiative.
I will if appropriate host a dementia awareness session for Yorkshire Housing staff and or tenants and their families, - to promote understanding and tolerance within the scheme.

Signposting
I would obtain and provide information to support a person with dementia and their family including signposting information to appropriate dementia support services .

Soth Yorshire Fire and Rescue Parnership
I will commit to become a South Yorkshire Fire and Rescue partner making referals for home safety checks( covering fire alarms and trips and fall risks etc) for the tenants of the scheme - recognising the most vulnerable groups and acknowledging the fact that a person with dementia will create at greater risk to themselves and others.

Dementia Friendly Homes - Physical Enviroments
I will gain knowledge about dementia friendly physical environments and visual perception issues that may occur in tenants homes that may cause distress and concern .
I will cascade this information to tenants and tenants families where appropriate

Worried About Your Memory campaign
I will display an Alzheimer's Society Worried about Your Memory poster and have some Worried about Your Memory leaflets available - I will encourage people to talk about their concerns and go to their doctor for a diagnosis whenever appropriate

Promotion Of Herbert Protocol
I will promote the use of Herbert Protocol ( an Information gathering tool for families to complete ) and the AZ society Help cards .
If appropriate and permitted by families I will store a copy of Herbert protocol within the scheme.

Promotion of RDAA
I will promote involvement with Dementia action Alliance in my local area to help to make the surrounding area to be dementia friendly.
If appropriate I will follow DAA on social media to help raise it's profile
