1. Action Plan

What we are going to do: All our staff will follow the National Dementia Declaration at Heathers nursing home and work as a team to transform the life for the people who affected by dementia.

We will provide training to all our staff to update the knowledge in order to provide a quality of life to all the residents who have dementia especially in our home.

We are and we will continue to deliver a person centred care to encourage residents with dementia to live well.

We will promote public awareness and understanding about dementia by providing information to the staff, families and friends.

Review effectiveness of these actions by audits, questionnaires and inspections and reports from different organizations like health watch, South Gloucestershire council and CQC. And   by conducting family meetings.
