1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The Centre will be designated Dementia Friendly and will run Dementia Friendly specific activities and advice sessions on Friday lunchtimes/afternoons available for residents across the London Borough of Lewisham and to specifically include the five wards covering Neighbourhood 3.

The Centre will run a continuous awareness campaign through social media and through all GP surgeries in Neighbourhood 3 to promote the Friday Dementia friendly sessions and will invite specialist groups such as the Alzheimer’s Society to attend advice sessions.

The Centre will work with its current partners, Lewisham Public Health, Lewisham Cultural and Community Development, Local ward assemblies, Lewisham Clinical Commissioning Group, local GP practices and pharmacies, Community Connections and Advice Lewisham to promote its dementia Friendly activities and target the hard to reach and seldom heard.

All mentors working on the Centre’s ‘Active Lifestyles for All’ programme will receive training to become Dementia Friends together with the community café staff and all management and a Dementia Champion will be appointed to co-ordinate all Dementia Friendly activities.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

To ensure all staff working at the Healthy Lifestyle Centre and on ‘Active Lifestyles for All’ are fully trained in what it means to be Dementia Friendly

To scope suitable trainers and coaches to deliver Dementia Friendly activities such as face yoga, stretching, Zumba Gold etc.
