We are a GP'S practice in Dalton ,Rotherham providing health care services for the local community . We have Approximately 2,062 patients and offer a wide range of services.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We are committed to making sure that all staff in the practice have Dementia awareness training and will encourage them to become “Dementia Friends”. Our GP’s and other clinical staff will actively engage in the Worried about your memory campaign with the aim of increasing diagnosis rates in Rotherham and we will ensure Carers of People With Dementia are registered on our practice “Carers Register”.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

As an organisation we must ensure that all our staff are involved in delivering a dementia-friendly service and to achieve this the correct training will be needed for all.

2. Actions

  • Diagnosis

    Gp’s actively engage in increasing diagnosis rates and consider using  the Worried about your Memory campaign- ask for details  –aim to increase diagnosis rates in Rotherham

  • Flagging records patient records and at Risk

    Flag patients records to indicate when a patient has Dementia and memory problems – smaller practices may consider new employees.

    Have conditions which are predisposed to individuals developing dementia on the At Risk register to provide a prompt diagnosis

  • Care for carers

    Ensure Carers of PWD are registered as a carer and support is offered and issues are looked out for.

  • Dementia awareness and dementia freinds

    Ensure all staff in the practice have Dementia awareness training – this is built in to training plans as essential

    Include concepts and guidance relating to Customer care from AZ Customer front  facing guidance booklet .

    Ensure all staff are signed up as Dementia Friends

  • Dementia friendly enviroments

    Ensure staff have knowledge of dementia freindly enviroments - Brookfield has been designed with dementia taken into consideration .

    We are going to have reminiscence  items in a quite area to further enhance this.

    We are also planning to have a memory tree .
