1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

  • Development and delivery of training programmes in dementia for nursing and medical staff
  • Creation of dementia friendly environments with  ward dementia champions, improved signage and orientation boards introduced in some areas (Pilgrim Hospital)
  • Guidelines for management of Behavioural  and Psychiatric Symptoms of dementia and delirium available for all staff on Trust intranet
  • General Hospital pathway for people with dementia  available on Trust intranet
  • Introduction of Alzheimer’s Society Hospital liaison  12 month pilot at Pilgrim Hospital to increase quality of patient and carer experience and care
  • Introduction of care home liaison service for people with dementia

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

  • Multiple site trust
  • Frequent turnover of trainee medical staff
  •  Nurse staffing levels  to allow protected time for training
  • Establishing  need for dementia to have high priority in trainee doctors’ induction and training programme
