1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

How will you make sure that people affected by dementia receive the best possible service and help?

By making sure all our staff have an understanding of dementia and how best to support those who are affected.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Making sure all staff complete the on line dementia training through AWP.  This could be checked at everybody’s annual appraisal meeting with their manager.

Arrange for all staff in Positive Step to complete an AWP online dementia awareness training.

  • Point of contact and where to find information

Create an electronic folder on our server where dementia information is stored for the whole team to access. In this folder would be suggested helpful books, electronic links to dementia information leaflets etc.

  • Working alongside Alzheimer's Society

Continue working alongside the Alzheimer's Society by running our carers courses at their luncheon club where the cared for is being looked after. This will mean more carers in North Somerset will have access to phychological support.
