1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The importance of the increasing number of people living with dementia and the implications for services within our local community will be actively discussed and promoted at the Medway VCS Leadership Network. All emerging actions will be duly identified and addressed by way of an action plan.

By becoming a member of the DAA, this will enhance the work of the DAA Action Group locally and give a voice to people living with dementia.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Actions would be reviewed via the Medway VCS Leadership Network on a 6 monthly review
