1. Action Plan

We work with and provide services specifically for people with dementia. We work with other local services such as a local dementia day service offering access to activities within our care home. We work directly with relatives and carers of people with dementia, and look to develop this further.

We will review the effectiveness of our outcomes through:

Feedback from our residents, their relatives/carers, and other professionals/stakeholders.

Internal reviews and audits.

External reviews, audits and inspections such as the Care Quality Commission, Local Authority and Clinical Commissioning Group.

Access and use of our service.
