1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Our trained skilled and knowledgeable staff offer services (personal care, domicillary care, be friending service, domestic care) to adults in the comfort of their own homes. Taking into account their individual needs and support requirement, our staff ensure the promotion of independence is key. Thorough assessments are carried out to ensure the services meet the needs of the client. Clients whether private or social funded are involved in decision making and encouraged to express their preference. we accept short, long term and live in packages of care. 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Lack of time or fundinga can be a challenge to delivering such services, the ability of our client may differ. We will therefore do our upmost best to encourage multi displinary meetings to make decisions which arr in the best interest of our clients.

    Personal assessments are carried out with the client or their loved one prior to implementing the support, this helps to tailor the support in line with client needs. Routines, preferences and needs are taken into account when agreeing services.
