1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Our organisation was approached by East Lancashire CCG to create a Memory Garden Project which could be accessed by members of the community who have a recent diagnosis of dementia.

This project would enable both the individual with dementia and their carers or relatives to engage in horticultural activities in a socially inclusive environment. We have volunteers and staff trained as dementia friends and have established links with Dementia Friendly Rossendale and a residential nursing home with a dementia unit.

Our ethos is to encourage interaction with wider social groups within a supportive setting , and raise awareness of the needs of those managing dementia.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Our challenges are around creating an accessible environment which is inclusive to all involved with the care of/ managing dementia.
