Salford Healthy Communities

Salford Healthy Communities is a community-led health and well-being improvement programme. We recruit teams of volunteers which are made up of local people, front line workers and other people that identify themselves as being from that community. We learn from one another, as well as providing each other with the support and local knowledge to make a difference. We are a Social enterprise with a strong social ethos and are currently commissioned by Salford Council to deliver the Long Term Conditions Prevention Programme

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We are committed to supporting dementia friendly communities initiatives in Salford through our services such as the Sleep Matters Project and also promoting the importance of dementia awareness.

2. Actions

  • Dementia Awareness through Dementia Friends

    Salford Healthy Communities has now 7 new Dementia Friends and 2 Dementia Friends Champions to help with this work.  The results of the project will hopefully inform and be rolled out elsewhere in the country. We also encourage and offer Dementia Friends Information Sessions to all our team members so they have increased awareness of dementia that will inform their work

  • Supporting the Salford DAA

    We will spread the word about Salford Dementia Action Alliance and the importance of dementia friendly communities throughout our work as an organisation. We will endeavour to attend DAA meetings and regularly update on our progress with the Sleep Matters project and others.

  • Sleep Matters Project

    We have been commissioned by Salford Council to deliver the ‘Sleep Matters’ project, this is a service for people living with dementia and their carers, who are experiencing difficulties with their sleep patterns. Our aim is to work alongside families using strategies and methods that could help improve their situation and lead to a better nights sleep. This is a pilot project involving volunteers.
