1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The practice provides counselling services for all people and this includes people with dementia offering emotional support and aiding acceptance of current circumstances and carers in offering full emotional support to enhance well being.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Counselling for people with living with dementia is a new challenge and we need to devise ways of measuring differents outcomes when counselling people with dementia.

2. Actions

  • promote dementia awareness training for all practice approved counsellors

to ensure that all approved counsellors working at the practice have had dementia awareness training and are therefore better equipped to provide services for those living with dementia

  • to ensure the profile of Dementia Action continues to be raised at the practice

we want to make sure that the work already done in the practice in raising awareness and provision of services for people living with dementia is continued, and that the profile of this remains high within the practice.
