1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We are signing up to the iSpace iniciate to ensure that we fully recognise and support patients with dementia. We wish to make significant changes to our surgery and train staff to ensure we are dementia friendly. Our carer's group is aligning with a dementia group locally.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We have found that training has not been great in the past, however now that we have been in contact with the iSpace people, we feel that significant strides forward have been made.
