1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The first thing I would like to say about the way we work is it is the same for all of our customer regardles of whether they suffer from dementia or not.

All of our clients are traeted with respect, have choices of carers and are 100% involved in planning their care. Our carers are all required to become dementia friends to raise their awarness of dementia. Our client centred care plans are reviewed every 3 months with a oportunity to be reviewed at the request of the client and their family at any time. We believe we supply a service that supports the clients to live their life the way they want to by producing a free optional customer specific booklet, which details their social activity preferences, hobbies, interests and family members they wish to visit. We also include diety preferences, things of interest for conversations. Our company tag line is "It's All About You" Our customer specific booklets enables us to research the local area and investigate activities that can be suggested to the clients to enable them to either continue to fuction as part of the community or begin a new ventur by becomming a integral part of the community

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We don't feel there are any challenges to us delivering these outcomes, it should be taken for granted that the 7 outcomes are being delivered by not only care providers, but for society in general.

2. Actions

  • Customer specific booklet

    We will ensure any of our clients with dementia are activly encouraged to complete the customer specific booklet to enable them to take full advantage of their choices of activities within their community.

  • Dementia Friends

    All of our carers are required to become Dementia Friends.

    We do not employ people who don't feel they are able to commit to this.

  • Dementia Champions

    We are currently organising 1 to 1 training for our carers to become energised to take the challenge of becoming dementia champions. We have negociated terms of business for a dementia specialist to train our carers and this will be completed for a sample of our carers within the next 3 weeks.
