Bradford District Care NHS Foundation Trust, Community Dental Service

The Community Dental Service is a specialist dental service providing care for patients who cannot have treament at a high street dentist due to medical problems ,disability or special social need. Dentists, doctors and health care professionsls can refer patients to the service through our office at Horton Park. We have clinics across the Bradford and Airedale district.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The Community Dental Service is currently working to become dementia friendly.  We have made contact with the DAA , signed up to the organisation and also joined the Alzheimer's society.

We have an action plan which is monitored by the service governance group and we have a lead person who is driving this forward.

We are raising awareness of Dementia with our staff by watching the useful information videos on Alzheimers website. Staff are undertaking additional training in Dementia supported by Bradford College . We are working closely with the Dementia Assessment Unit at Lynfield Mount to promote awareness of dental care and the treament available for patients who attend the unit.

Our operational staff bulletin promotes additional training, information and events staff can attend.

We are currently assessing our clinics to improve the environment for patients with dementia .

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Our challenges are that we are limited to what environmental changes we can make as we do not own the buidlings we work in.

Dementia is only one of the special care groups the service treats therefore we have to balance changes and the possible impact they may have. However , some changes will benefit all the groups we see.

2. Actions

  • Dementia Awareness session for the dental service staff.

    Training to be identified and provided at staff meeting in April 2016; DAA contacted to request a ‘Dementia Friend’ to attend the meeting.

    74 members of the dental team attended the Dementia Friends session in April.

  • Dental Care Professionals enrolled on educational course to obtain NCFE in Dementia and Mental Health, run by Bradford College.

    Staff have obtained the qualification

    Staff to attend sessions on the Dementia Assessment Unit to work along nursing staff to broaden their experience of working with patients with Dementia.

    Staff have now obtained qualfication inn Dementia and Mental Health. Staff attended the DAU to underpin their academic eduaction.

    Complete the Kings Fund ‘Is your health centre dementia friendly’

    EHE Environmental Assessment Tool.


    Review the premises using the Sterling University model.

    DAA group to visit sites in October to make recommendations .

  • Ensure people living with dementia are consulted about suitability of site/ service.

    Approach FIT Group (people with dementia) to ensure environment meets their requirements.

    HT to attend FIT meeting in October 2016 to meet the group and discuss assessment of the site.

  • Make contact with the Alzheimer’s society and Dementia leads within the organisation/ area; share best practice to develop DFDS

    Service registered on Alzheimer’s Society - Dementia Friends.

    Tool kit obtained to become a Dementia friendly service.

    Email to all staff to signpost them to the videos on the AS site. 

    Registered with Dementia Alliance Association and met with Bev Fletcher to discuss action plan.

    Met with Jacquie Edwards Lynfield Mount Dementia Assessment Unit Manager 5/1/2016. Collaborative work under way to share best practice and training. 

    Oral Health Asseessment template developed with the DAU. Training for doctors, nurses and support staff underway with annual updates planned.
