Yorkshire Housing - Stay Put

Support, advice and home repairs, improvements and adaptations to help older or vulnerable people live at home safely and independently. In Rotherham the first hour of labour from the handyperson service is free if you are an older or vulnerable person. We are a not-for-profit Home Improvement Agency operated by Yorkshire Housing in partnership with Rotherham Metropolitan Council.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Our organisation can help support people with dementia and their families by increasing the teams awareness of Dementia and the difficulties our service users may be experiencing. Some of these difficulties will have direct likes to the services we provide.              

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Ensuring all of our team has dementia knowledge and embedding this knowledge in our mandatory training systems

2. Actions

  • Dementia Awareness and Dementia Friends

    We will commit to provide a dementia awareness / dementia friends session for our whole staff group – incorporating a questionnaire to gain knowledge of peoples own personal involvement with dementia.

    We will sign up as an organisation on the Dementia Friends website and embed the 5 information video clips into SYH staff training programmes.

    We will cascade the A Guide for Customer Facing Staff within our team

    We will give consideration to our employees acknowledging the personal effect of dementia with families

  • Dementia Friendly Environments

    We will ensure that our team has information and knowledge about dementia Friendly environments and show commitment to cascade this information to families where appropriate

  • Sign Posting

    We will ensure our team has knowledge about local support services available from Alzheimer’s Society have a supply of leaflets and sign post to relevant services when and where appropriate

  • Herbert Protocol and SYFR home Safe and Well home visits

    We will ensure all staff have knowledge of Herbert protocol and encourage families to use it as an information gathering tool to minimise distress for the family and maximise efficiency of the police.

    SYH will re- engage with South Yorkshire Fire and Rescue to ensure that the Safe and Well home checks are a standard referral point acknowledging that a person with Dementia will be a greater risk of accident.

    These visits will enable us to cascade knowledge of Stay put services helping people live well with dementia in their own homes as long as possible. sessions are booked in at three of the memory cafes
