1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

To help those with Dementia and their carers to enjoy a supportive environment where they feel valued and understood, and to faciliate a sense of belonging and of being a valued part of family community and civic life we xare committed to the following:

We run a carers support group and support Keystone; our local church charity for Dementia carers and cared for. We have had dementia friends training and approximately 30% of our worshipping congregation are dementia friends. We have an effective Safeguarding policy and training to support Adults at Risk. We do dementia friendly worship in our two local nursing homes; responding to residents preferences for hymns and music. This year we have done our first memorial service there, we also provide "twiddle muffs" .We have dementia friendly songs of praise services.The 10.30 services all have bespoke service books which are easy to follow. 

We are working with the Alzheimer's Society and Dementia Action Alliance to enable the Diocese of Liverpool to become a member of the alliance 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We are a largely voluntary body with porous boundaries between church and community; this helps us connect locally but means we act through facilitation and encouragement, not proscription.

Our building is in need of improvement to be totally dementia friendly. 

2. Actions

  • An evening carer support group

    We plan to start a carer support group, which will be in the evening, for carers who need to go out to work or have other commitments during the day. This will start on 1st March 2016 and will be held monthly.

  • Encourage local businesses to be more dementia friendly

    We will work with Keystone and Alzheimer’s Society to influence local businesses to be more dementia aware and friendly, for people in the community who are living with dementia.  We will hold an evening Dementia Friends training session for local businesses and give personal invitations by the end of December 2016

  • Raising dementia awareness in the diocese

    We will work with the local Dementia Action Alliance and the diocese of Liverpool, to improve dementia awareness and friendliness in the diocese.

  • Making our premises more dementia friendly

    We will try to make our building development as dementia friendly as possible, both in signage and the organisation of the premises.  We are in the final stages of planning and funding an extensive reordering and extension of our premises and will use local dementia champions and carers to assist us in our planning and furnishing. The building development is planned for 2017
