Dr S P Singh and Partners

Dr S P Singh and Partners is a GP surgery practicing from modern, bespoke premises offering a wide range of services. We provide healthcare for our 9500 patients from South Kirkby, South Elmsall surrounding areas, we also have a branch site in Hemsworth. Our team includes 3 GP partners, 1 Salaried GP, 2 Nurse Practitioners, Practice Nurses, Health Care Assistants, District Nursing, Health Visitors, Midwife, Counselling & Physiotherapy, In addition, we have Ultrasound, Audiology, Health Trainers and screening services available to our patients. The whole team work together to deliver excellence in primary care, we also have a Patient Participation Group who meet regularly to help continually improve services.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We will discuss your care needs and wishes with you, to ensure you have choice and can influence any decision that are made regarding your health and wellbeing.  As part of ongoing care, we will endeavour to provide you with the information and access to services to best suit your individual needs.

The GP's, nurses and all staff at Dr Singh and Partners have completed Dementia Friendly training to support our patients and their carers and families to provide a supportive environment. One member of our Patient Participation Group has also trained to become a Dementia Champion. The surgery will continue to work with our community, hospitals and care providers to provide seamless and personal care together with you.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Delivering Care:  Increasing diagnosis of Dementia needs effective and timely diagnosis and referral pathways being available.

Require:  The integration of IT systems and communication links between all care provider organisations in delivering a complete medical record.  This would ensure all the elements of the patients care and any plans are available to inform and optimise care.

Raise Awareness:  To inform the general public of the services and options for support that are available.  Suggestion:  Local Alzheimer's Society could provide a coordinated list of resources and offer Patient Workshops to raise awareness.

Resouces:  To improve surgery to become a "Dementia Friendly" environment.

2. Actions

  • Improve the Surgery Environment

    We have completed Kings Fund "EHE Environmental Assessment Tool" with Patient Representatives.  We analysed the outcome with local Alzhiemer's Society and implemented changes within the surgery to make it a more comfortable environment for patients with Dementia and their carers.  Share with PPG group and patients via Website.

  • Dementia Friendly Communities

    The practice will organise an event at the surgery for anyone in our community to attend to raise awareness and educate the public about Dementia and how they can become "Dementia Friends". Invite other "agencies" to attend e.g. Carers Wakefield and Age UK.
