I am a self-employed personal assistant. I work with a lady who has dementia I work with another personal assistant we aim to provide quality person centred care. I work 5 days a week.

1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
I support the lady who I care for and her husband.
I aim to help her live as well as she can during all the stages of dementia.
I try and ensure that she still has social interaction and attend the Alzheimer’s society memory cafes the aim being to ensure she is still simulated
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
I have had to learn about dementia as a disease of the brain to enable me to do my job professionally.
I am aware that dementia is a progressive condition and the needs of my client will change and that I must adapt to this change.

Dementia Friends and Dementia Freindly Communities
I have signed up as dementia friend on line and encourage others to do so too .I will wear my dementia Friends badge to promote dementia friendly awareness in the community.
I will tell other people about Rotherham Dementia Action Alliance to help promote dementia friendly communities and explain to others about the working towards being dementia friendly logo
