1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

LiveWell support, encourage and empower individuals to promote healthy life styles and change habits so as to live healthy, in addition to referring them to other available services if needed, like Sutton UPlift and Everyone Active.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Outreaching people in need for the services like hidden carers and people with dementia.

  • Outreaching individuals in need

    We will be attending event that DAA  and Sutton Carer Centre organised so as to out reach individuals in need.

    Being implemented
