1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

  • Individualised care through the use of Personal Information for the Patient with Dementia (PIPD) assessment documentation.
  • Dementia Policy and pathways are in place to facilitate effective and compassionate care.
  • Programme for staff at all levels within the organisation to be appropriately trained in dementia care so that the patient focused individualised care can be provided.
  • Specific dementia link staff identified in all clinical areas across the trust
  • Trust wide support group for dementia link staff.
  • Multi-disciplinary privacy and dignity group ensure that values are maintained
  • Patients with dementia are identified as part of the pathway and staff made aware prior to visit
  • Active participation in the London Acute Hospital Dementia Network

 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

  • Monitoring the impact of good quality dementia care across the trust.
  • Ensure that processes are consistently applied across the trust and accessible services are available in all areas.
  • Creating a suitable environment within the physical constraints of the current setting
  • Ensuring that all staff are educated with regard to dementia within the current resources
  • As an ambulatory care service developing awareness and skills when patients visitors are transitory
