Good Measure Pharmacy

We are a community pharmacy- but with a difference . We use training dispensers or a pharmasacist to deliver paitients medication . We cna answer any queries or problems at the point of delivery .We also ensure our paitents are complying with their medication dosage and we can help with dosage problems . We provide MDS trays that help people whi may be having difficulty remembering to take their medication .Our trays are colourcoded and carry pictures of the medication and a photograph of the paitent . Together with Social Services we will provide the pivotell atomatic pill dispenser and will help our paitents to set the system up and explain how the system works - on a weekly basis. We also work closely with Social Services ,local Gp's ,family members ,the NSH ,intermediate care sercices ,care agencies both private and council,Rotherham hospice to help patients take their medication as precribed by their Gp. We provide a bespoke Pharmaceutical service we are proud of .

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The service we provide take into account the difficulites experienced by a person with dementia. We look at solutioins to assist and remedy problems that a famly are encounting.

Our service is person centred and we do very much more than deliver medication - we are committied to give out patients time and consideration to help them feel comfortable and reduce their anxiety .

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The biggest challenge we have is getting information about our quality service known so we can increase the number of patients we care for to keep people living well with dementia.

  • Dementia Awareness

    Ensure all staff have had Dementia Awareness training .

    This can be sourced free of charge from The Social Institute of Excellence – see attached .

    Embed this into you staff training for any new members of staff .

    Ensure all staff become Dementia Friends via the website

    In addition Register as an organisation on the Dementia Friends website and gain access to 5 awareness video clips – ensure staff watch them

  • Dementia Friendly good practice

    Cascade your dementia friendly good practice using RDAA to make others ( including GP’s and RDGH) aware of your services which are dementia friendly .

    Individual contact and advice at home and the dementia friendly Monitoring Dosage system.

    Gain Knowledge of AZ society support services and information and sign post whenever appropriate – stock some information that can be given to people wih Dementia or their families
