Dementia Support

Dementia Support aim to deliver seamless and holistic care from a centre of excellence for local people living with dementia. Offering support to those with and without a formal diagnosis of dementia we will work in partnership with the NHS, West Sussex County Council and other statutory, voluntary and charitable organisations from within the community. We will: • provide a centre of excellence to support people living with dementia • consolidate dementia services in the area into one location • be an integral part of the community • be a local charity for local people • offer training and education and become a beacon for best practice in the provision of dementia services

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Working from one central location, Chichester Dementia Support Group and their partners will:

reach out to around 5,000 individuals and their families living with dementia in our local community by 2017

offer person-centered information, guidance and support at a level that suits each individual

ensure those living with dementia are treated with dignity and respect and are listened to at all times

work collaboratively with statutory services including public health to build trust, respect and confidence in the support that is available ensuring we are accessible for hard to reach groups of people affected by dementia

raise awareness of dementia within the wider community including undertaking research and evaluation/ all our staff with be a dementia friend and we will offer dementia Freinds sessions to the local community

provide an age appropriate service to those with Young Onset Dementia tackling specific issues faced by this group

assist the workforce supporting those affected by dementia to share and create best practice in the delivery of care through bespoke training

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Working in partnership with other 3rd sector and statutory services to bring all dementia services together under one roof

  • raise awareness of dementia within the wider community

    All staff and trustees of Dementia Support will be a Dementia Friend. Some staff will become Dementia Champions. we will offer and provide Dementia Friends sessions to the local community

    Working individually and collectively with other dementia providers, both 3rd sector and statutory. Working towards signing a partnership agreement

    Set up a befriending system run by paid staff as well as volunteers to support those living with dementia.  This will start by offering information and advice and the aim is to develop into a true befriending service
