1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The simplest and most effective way for Meltham Road Surgery to make a difference, is to increase awareness of dementia within all our practice staff via Dementia Friends.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The biggest challenge for Meltham Road Surgery is time. By Meltham Road Surgery working in partnership across GHCCG, with other health and social careproviders in Kirklees, we hope to be able to make steady progress in improving what we do with all patients.
