Fareham Community Hospital

For the past 14 years we have specifically targeted the Health Market and we manage over 100 health facilities. We are keen to ensure our building environments enhance patient experience and understand specific needs of people with dementia. We want to role out a programme to make all our properties Dementia Friendly.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We work with the physical building layout to improve the patient experience of people with dementia and their carers. We are pro-active on making changes where we can to achieve this goal.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The main challenge is proving to Health Care Providers that the Physical changes we carry out in buildings are extremely worthwhile.

2. Actions

  • Dementia Awareness raising for staff

Staff and Friends of Fareham Community Hospital have attended awareness training

  • Awareness of the hospital environment and how it may affect people with dementia.

Our staff’s awareness of things which may cause issues but which are impractical/financially unfeasible to change, will allow them to assist people living with dementia as they move around the hospital.

    Status:

  • Signposting at Fareham Community Hospital

Signposting is being made to comply with observations from dedicated staff assessments
