1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
Person centred care plans to enable choice,support and control on how wish to live.
Referrals to ensure early diagnosis via Gp.
Comprehensive acess to information to make informed decisions on how person wishes to live.
Enable decision making on care received.
All carers trained in Palliative care.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
Limited understanding from relatives.
Delayed response to referrals.
Poor response / support from community.
