1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
Our services include enhancing the wellbeing of people with dementia and their carers
We would hope to increase the referral numbers of DWD from Gp’s and to be more knowledgeable about the difficulties a person with dementia may have.
In addition by working more closely with RDAA and Rotherham Alzheimer’s Society we would hope to cascade more knowledge about our services which would enable people to self-refer to some of the services.
By ensuring all our teams have Dementia Awareness knowledge we will be able to provide a dementia Friendly service.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
It will be vital to ensure all the teams have dementia awareness knowledge and embed the cascading of this knowledge in our training systems to ensure that is sustained over time
