1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

To fully understand challenges faced by those with dementia and be able to respond to their needs by providing a welcoming environment and staff who are able to assist.

To improve the way we care for those affected by dementia.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Availability of staff for training, ability to change our building.

2. Actions

  • Dementia Awareness raising for staff

    Ensure all staff, attend a dementia training; Dementia Friends awareness raising session and ISPACE training as appropriate

  • Environment

    Review of our environment through ISPACE – look at dedicated board in waiting room, improved signeage, dedicated folder in reception with dementia resources

  • Partnership Working

     Working with carers and other agencies using ISPACE – look at possibility of carer clinic

  • Care Planning

    Look at all patients with dementia read code and encourage advance care planning

  • Information

    Increase awareness of available resources for patients with dementia and their carers – possible circulation of amalgamated info to all coded patients
