1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Essex County Council has a lead and pivotal role in delivering better outcomes for people with dementia and their carers. Due to the complex landscape within Essex, namely 7 CCGs, 12 districts councils and 2 unitary authorities, a fundamental part of Essex County Council's role in this is to:

Raise the profile of dementia across the County to ensure engagement, particularl at a senior level within organisations, across the health,social care, private, public, commercial sectors.

Lead and shape a system wide approach to transforming the lives of people living with dementia

Co-ordinate activity across the county to ensure practical actions are being taken on a county wide and local level to deliver better outcomes for people living with dementia

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Awareness and understanding of dementia across the orgnaisation and in the wider community

Engagement from key players across the Commercial, private,health,social care, public and third sector at a county wide level

Engagement from local areas to see dementia as a priority area

A complex landscape of different communities and different demographics

To sustain the momentum of a strategic alliance in taking practical actions to transform the lives of people living with dementia
