1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We provide a caring environment for members of our church and have good relationships with various other groups in the village. Numerous community groups use our premises and we are part of a good communications network in the community. We can raise awareness of the disease and its effects on those caring for sufferers. We can provide an environment physically suitable and emotionally supportive.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We need to provide awareness training to more of our church members, and a suitable physical environment. 

2. Actions

  • Dementia Awareness raising for staff

    Ensure all staff attend a dementia awareness raising session

  • Raise dementia awareness with the wider church

    We now have a dementia champion who is in the process of delivering the awarenss sessions to others in the church.

    Provide awareness training to the team visiting nursing homes.
