We are a Christian church who support those in need and work actively in the local community. We help charities nationally and internationally. We are active in helping people in all stages of their lives. We support carers and those with Dementia to live well, and access support through faith.

1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
iT IS IMPORTANT TO THE CHURCH TO KEEP INFORMED OF CARING FOR THOSE WITH DEMENTIA, SO WE CAN CONTINUE TO SUPPORT THOSE OF OUR CONGREGATION WITH DEMENTIA, AND SUPPORT THEIR CARERS.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
THE ONLY CHALLENGES MAY BE IF WE HAVE NEW MEMBERS JOIN THE CONGREGATION WHO HAVE NOT ATTENDED DEMENTIA TRAINING. WE WOULD REVIEW WHETHER WE WOULD BENEFIT FROM FURTHER TRAINING IN THE FUTURE.
ACTING ON DEMENTIA AUDIT.
WE INTEND TO SECURE ALL LOOSE MATS IN THE CHURCH CENTRE AND IMPROVE THE SIGNAGE TO THE TOILETS, MAKING IT DEMENTIA FRIENDLY.

IMPROVING FAITH RESOURCES FOR THOSE WITH DEMENTIA.
TO ORDER SOME LARGE PRINT BIBLES AND CONSIDER OTHER RESOURCES BEING IN LARGER CLEARER PRINT, LOOKING AT REVIEWING WORSHIP RESOURCES.

TO KEEP WORKING WITH THE LOCAL DEMENTIA FRIENDLY COMMUNITY STEERING GROUP.
FOR THE CHURCH MEMBER TO KEEP THE STEERING GROUP INFORMED OF ALL DEMENTIA FRIENDLY ACTIVITES THE CHURCH IS HOSTING.
TO PUT ON A DEMENTIA FRIENDLY CHURCH SERVICE AND ADVERTISE THIS TO THE STEERING GROUP, THROUGH THE COLEHILL PARISH MAGAZINE, THE LOCAL LIBRARY AND HOPE FM IF POSSIBLE.
