Unsworth Medical Centre

Unsworth Medical Centre is a general practice (GP) surgery at the heart of the community in Unsworth, Bury, delivering high quality healthcare to local families. In addition to being a general practice there are many clinics for the management of chronic diseases such as asthma, copd, diabetes and heart disease. We also offer a wide variety of other medical services including HGV/Taxi medicals, minor surgery, joint injections, contraception services, drop in sexual health clinic, childhood vaccinations, flu vaccinations, organisation of blood tests and and well-person check-ups.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Providing a dementia friendly patient service and environment to encourage people living with dementia to visit our premises and feel comfortable here. This will improve their confidence and well-being and help to reduce the stigma which still exists around dementia.

We are also promoting awareness of dementia and strategies to enable well being in dementia.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Lack of undesrtanding amongst our staff and other patients about the challenges experienced by people living with dementia and their carers, and what we can do to make life better for everyone.

2. Actions

  • Improve staff awareness

    Encourage all patient facing staff to complete the on-line Dementia Friends registration and/ or attend a local Dementia Friends / Customer Awareness session and provide access to the booklet 'How to help people with dementia -a guide for customer facing staff'

    Review the customer environment checklist provided and make any immediate changes we can now, noting those we can make at the enxt refurbishment.

  • Support the Dementia Friendly Communities campaign in the area

    Join the local Dementia Action Alliance and display the Dementia Friendly Communities logo publically, informing patients why this is important.
