1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Ensure all staff  have a good awareness and understanding of people living with dementia. The Transcare management team will monitor itself and all other members of staff to watch for early signs of dementia. Any Transcare employee  who is actively caring for a family member living with dementia will be given extra support.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The challenges will be to continually update staff on the subject matter.

How can you overcome these challenges?

The Transcare Management team will continue to keep refreshing it's knowledge and understanding of dementia and institute new initiatives where and when appropriate

All drivers will have refresher training by Bradford council to keep their knowledge and understanding of dementia at the highest level possible.

Drivers will endeavour to form a good working relationship with patients, carers and families. Where possible drivers/escorts will be used for the same passenger living with dementia, so the crew can become familiar with the requirements of all concerned. When this is not possible crews will be expected to share knowledge of passengers, carers and families, to ensure continuity of high quality service.
