Curo Health Limited

Curo Health Limited is a Federation of all 29 GP practices within the North Kirklees area (exactly the same coverage as NHS North Kirklees Clinical Commissioning Group). GP Federations are a relatively new type of organisation that are set up so that GP practices can provide services in a collaborative manner. Our main area of focus at the moment is the co-ordination and provision of two schemes that are targeting the needs of some of the older and more vulnerable patients within the community.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The simplest and most effective way for Curo Health Limited to make a difference, is to increase awareness of how the GP practices it represents engage with the public. 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The biggest challenge for Curo Health Limited is time. By Curo Health Limited working in partnership with it’s federated practices, alongside NKCCG, and other health and social care providers in Kirklees, we hope to be able to make steady progress in improving what we do with all patients. 

2. Actions

  • Improve our understanding of Dementia

    Curo Health Limited will encourage member practices to get their reception/ admin staff to watch the Dementia Friends film at www.dementiafriends.org.uk or attend a face to face Dementia Friends session.

    For those who are interested to know more, we will encourage them to participate in the eLearning resources at www.scie.org.uk/publications/elearning/

     We will ensure all practices have access to: Making Kirklees a Friendly Place to Live Toolkits, Dementia Friendly Kirklees Resources. Details can be found at: https://www.kirklees.gov.uk/community/careSupport/healthWellbeing/pdf/dementiaToolkit01 .pdf

    We will co‐ordinate a Dementia Friends information session once a month in a different Health Centre/ GP practice each time. We hope this will support broader awareness, and accessibility for GP practice’s patient reference groups, individual patients, carers and member of the local community. 

  • Raise awareness of dementia

    • Curo Health Limited actively focuses on the needs of patients within North Kirklees, and hopes to become an organisation that is working towards becoming Dementia Friendly.

    • As staff within GP practices across the Federation watch the Dementia Friends video or attend a session we will encourage them to proudly wear their badge.

    • If resource allows, support any local developments co‐ordinated by Kirklees Dementia Action Alliance, NHS North Kirklees Clinical Commissioning Group, or other interested party to work towards Curo Health Limited and it’s member practices being part of a Dementia Friendly Community.

    • We will provide a link on Curo Health Limited’s website to allow people to find out more/ book onto Dementia Friends sessions for themselves.

    Working alongside our member practices,NHS North Kirklees Clinical Commissioning Group and other colleagues, organisations and individuals within Kirklees, Curo Health Limited will review and reflect on how further changes and improvements might be made for the benefit of all patients.
