1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Our staff see many patients from their local communities on a daily basis and are able to build relationships with not just patients but also their families and carers.  Being a local, familiar face in the community, our staff are able to offer a friendly place for patients to talk to and get reliable, expert advice to help with their health and wellbeing.

They are able to recognise the signs of patients experiencing degrees of memory loss and are ideally placed to help provide support to patients and carers with their medication.

 Services offered to help in this way include prescription ordering, collection and delivery.  We are also able to provide support help patients take their medication safely and reliably in their own homes, with or without the help of family or carers.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Given our long standing presence in many of the communities we service, our staff are already very visible and known to members of the local community and are well trained and prepared to have health related conversations with patients, family, and carers.

 Cascading training to our 200 staff will be the biggest challenge.

2. Actions

  • Training Plan

    Develop a training plan to direct and cascade Dementia Friends training throughout our 20 branches to 200 staff.  Identify those staff currently registered as Dementia Friends or Dementia Friends Champions to help act as advocates.

  • Managers Meetings - Information

    Introduction for managers regarding plans to cascade Dementia Friends training to all branch staff and delivery drivers.   

  • Cascading of information

    Distribute training materials to branches and monitor progress.
