1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Sentinel as the GP provider for the Derriford footprint is key to delivering first class preventative services. We are working with this cohort of patients on a daily basis and have been asked by Parliament to report on some of the work we are doing. We see part of our role to raise awareness and improve services and will continue to work on this

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The challenges for us are twofold, firstly funding-there is a lack of funding in the system to provide a proactive, preventative service and we are constantly reviewing this situation.

Secondly, awareness at a national level of the issues that affect people's daily lives and the chance to influence government should be increased if at all possible.

2. Actions

  • We are hoping to get an early Mild Cognitive Impairment (MCI) service commissioned as well as the additional support/signposting that comes with it.

    We are working with Devon commissioners to commission an early diagnosis Mild Cognitive Impairment (MCI) service that will provide patients with support and signposting at a much earier stage and allow them to get the help and support that may not be available until their condition significantly worsens.

  • Become Dementia Friendly

    Sentinel as an organisation has become Dementia Friendly and all staff have now completed the Alzheimer's Society Dementia Friends awareness sessions. we envisage that this will help us provide better services for patients and help staff understand the issues they may be going through.

    We have appointed Dr Peter Williams to be Sentinel's Dementia Champion. He will take on this role and champion the cause both internally and externally to ensure that this is at the forefront of everything we do.
