1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

I would endeavor to identify a person with dementia and deal with them in an appropriate professional way demonstrating care and sensitivity

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

I may be approached to do work that is not required and may have to consider the need to talk to other family members .A customer with dementia may become confused about what they have asked for or what I have told them I am going to do or when I am coming to do the work

2. Actions

  • Dementia Awareness

I will improve my understanding about dementia and the understanding of my one employee .

We will sign up as dementia friends

  • Signposting

If ever appropriate I will tell families about the local services available to people with dementia and their carers .

My wife works for the Alzheimer’s Society so I have knowledge about the services available in Rotherham

    Being implemented
