1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Action 1:

Title: Raising an awareness of dementia in the wider community

We are committed to raising awareness and a better understanding of dementia.  This is achieved by providing up to date information, through leaflets, events including dementia awareness week and the roadshow bus, talks, stands and inviting people to become Dementia Friends and Dementia Friends Champions.

Action 2:

Title: Working in partnership

We are committed to working with partners across the community to share resources, avoid duplication and deliver cohesive, joined up services to people with dementia and their carers in the community. We have a partnership arrangement with Devon Partnership Trust to co-deliver post diagnosis sessions and Memory Matters education sessions

Action 3:

Title: Involving people with dementia

Description:  We continue to consult with people with dementia and/or their carers regarding services that they receive. We have an established leadership groups in Torbay and Plymouth which are for people with a diagnosis of dementia to take an active role in influencing local activities both strategic and service delivery wise. We are looking into a potential partnership opportunity in order to establish one in the Exeter and East Devon area.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

There may be some challenges around capacity for local staff to attend regular meetings. However, we will endeavour to support the local alliance where we can.  This will include providing information.  We are committed to working collaboratively with partners in the local community to achieve better outcomes for people with dementia and their carer’s in the community.  All Alzheimer's Society staff are Dementia Friends.
