1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We are a practice with over 13,000 registered patients of whom about 100 people with a diagnosis of dementia.

We provide medical care to or patients, physical, psychological and social.

 We undertake an annual review of all patient with this diagnosis and also will address any needs as they arise.

 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

In the patient it has been getting access to local services – older peoples mental health, respite, and other support.

 We are working closely with the Older Peoples mental health service, the voluntary sector – mainly the Alzheimer’s society and also the clinical commissioning group (CCG)
