Carlton Lane Pharmacy

Community pharmacists were known in the past as chemists. Like GPs, community pharmacists are part of the NHS family. Every day about 1.6 million people visit a pharmacy in England. Community pharmacies are situated in high street locations, in neighbourhood centres, in supermarkets and in the heart of the most deprived communities. Many are open long hours when other health care professionals are unavailable. There are several different types and sizes of community pharmacies, ranging from the large chains with shops on every High Street or in edge of town supermarkets, to small individually owned pharmacies in small communities, in the suburbs and often in deprived areas or rural settings. The traditional role of the community pharmacist as the healthcare professional who dispenses prescriptions written by doctors has changed. In recent years community pharmacists have been developing clinical services in addition to the traditional dispensing role to allow better integration and team working with the rest of the NHS.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We are a community  pharmacy so we are well placed to come into contact with those who have or care for those that have dementia.

We are here to support those people who are newly diagnosed with dementia and explain any treatment they may be prescribed as well as explaining why they may not receive treatment in certain circumstances.

We are also in a position through seeing people regularly to recognise signs and symptoms which may need referral to the GP to aid early diagnosis and provide signposting to services.

We are also able to help the patient and carers manage both of their medication needs whether it be long term conditions and advice. This may be through ensuring they have all the information or it may be through managing their prescriptions for them or packing the tablet into compliance aids.

We are also a small store with a seating area and consultation room if a patient needs a quieter environment. We also have the advantage of knowing our customers and therefore being able to help the without prompting.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

A community pharmacy can be a busy place and at times a very full shop may be intimidating. It is difficult for us to predict quieter times which may be more appropriate if the patient may become anxious. At present our consultation room is not situated off the shop floor directly which means it isn’t seen always from the shop area.

Trying to gain the right balance with patients and carers can also be tricky knowing whether someone is able to collect prescriptions for themselves or if there is a risk of in advertent medication overuse. Making sure every member of the team knows how to deal with these situations and which situations exist for which patient can be tricky.

    We are currently undertaking a re-fit of our internal lay out with some structural work. This is allowing us to site the consultation room in a more obvious place should someone need a place to wait off the shop floor.

    We are going to move the counter which makes a much more open space and much clearer areas for handing in prescriptions and gaining advice

    The seating area is also to be improved.

    Signage within the shop is also to be improved and clearer.

    The shop itself is small and entry is through a main door which is easily accessible

  • Staff and Team development

    The pharmacist has already completed the CPWY training session.

    Although given the nature of the work that we currently do I have been able to see the teams attitude and understanding of those with dementia and believe them to have a high level of understanding.

    In order to help make sure there are no gaps in their knowledge we will complete the Dementia friends training videos. Following this it may also be appropriate to train a dementia friends champion.

  • Optimising Medicines and patienmt support

    As community pharmacy we are very well placed to help support patients and carers with all their medication needs.

    We can provide support to those prescribed new medication.

    We can also help them manage their long term conditions unrelated to the dementia. As well as helping with the health of carers.

    We are also in a position to provide prompts, our own compliance aids and those provided by Leeds council such as pivotel. Where required.

    We can also provide support in helping carers organise medication through prescription ordering service.
