1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Age UK Lincoln provides a holistic service for older people which can be accessed at all stages of their journey into later life. People can access our Park street activity centre at any stage of their lives, but should support  be needed to continue with the activity, we can arrange for a day support worker or carer, to accompany the person. If further support is needed than can be offered at Park Street our two social outreach centres are available. Day support services can be offered to assist with household chores, gardening and laundry. If personal care is needed we can provide a service with well trained carers. Our Information and Advice department can ensure that the person is in receipt of all appropriate benefits and give information on a range of other services and organisations that may be able to assist the person. Our Carers Service can offer support and practical help to the person’s family including Carer’s assessments, as well as putting emergency plans in place. Our Technical department can ensure that the person has all the technology needed to ensure that the person can remain in their home for as long as is possible.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Park Street employs a number of volunteers as well as paid staff. We have to ensure that all our staff have a good understanding of the needs of those living with dementia.

2. Actions

  • Staff and volunteer training

    All volunteers and Staff will have undergone Dementia Friends Training as a minimum. Salaried staff in more strategic positions will undergo more intense training.   



  • Ensure buildings are dementia friendly.

    We underwent a dementia audit form an external agency which highlighted some areas that needed to be addressed. We looked at our signage, leaflets and website to ensure that they were as dementia friendly as possible. We looked at the building as a whole, floor coverings , paintwork bathroom fitting and access. Plans are in place to follow the recommendations made where possible. Some recommendations have financial implications that may delay their implementation.

  • Ensure that our activities a as inclusive as possible

    Looking at things like large game pieces and boards. Ensuring that activities within our social outreach service are on view and can be accessed at will.  Ensuring that the instructors of activities have awareness of dementia and recognise the need for inclusiveness.
