1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

 The company is in the process of arranging a ‘Dementia Friends’ session for staff and choir members so that we can better meet the needs of people living with dementia and their carer’s.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

  • Dementia Awareness raising for staff

    Ensure all staff attend a dementia awareness raising session

    2014 - Fourth Quarter Update

  • The company are organising a concert which we are making accessible to people living with Dementia.

    Being held at an inclusive venue that is Dementia Friendly Milford Community Centre. Alzheimer’s Society Dementia Support Manager will be available to support anyone in the audience with additional needs. Our choir will by then all be Dementia Friends.

  • Attending Milford on Sea Music group

    Choir Director attending a music group to give a short talk about the forthcoming concert welcoming all the clients to the event. This will encourage inclusion with in the community.

  • Making adjustments to support anyone living with Dementia to remain an active part of the choir.

    We will adapt music in larger print, provide support next to a person and if necessary use visual communication.

  • Support Milford on Sea Dementia Action Group.

    We are holding the concert to raise vital funds for the group to continue its work in the community of Milford. We are performing at the family fun day for the launch of Dementia awareness week on 17th May
