1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We have a person centred approach which enables us to adapt to the needs of the individual. With the correct support and understanding we help them make their wishes known and remain as independent as possible within our community.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Being consistent and having the time to devote to the person living with Dementia.
