1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We keep people informed about services and events happening locally that they can access we have one long term condition group that welcomes people diagnosed with dementia and are hoping to set up more of these peer groups that meet monthly. We are also looking at setting up personalised playlists of music for people with dementia that they can keep to listen to whenever they wish. We work with the local carers organisation as well as many others who provide care and support for people with dementia and their carers.  We promote dementia friends.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

2. Actions

  • Music for life

    Equipment obtained and downloading of music available on demand for people who want to use .  First client has received their  MP3 player with 10 tunes loaded.  Will now be rolling out publicity


    2016 - First Quarter Update

    Have obtained some donated MP3 players - getting them tested and obtaining appropriate headphones.  Will load music onto one as a demonstration and will be asking people who would like to try them to contact us in our next newsletter which is due out in December.

  • Encourage Organisations to join DAA

    We work with various organisations including housing associations and we will be encouraging them to join the DAA.
