1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Active Life aims to support people living with dementia and their carers using a dementia specific activity and education based programme.

Encouraging and maintaining activity as part of a daily routine and to enhance the physical and emotional wellbeing of people living with dementia and their carers in the local community

Active Life aims to continue working in partnership with the Local Authority, Public Health and our partners at the Southend Dementia Action Alliance to make Southend on Sea a Dementia Friendly Community.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Understanding Dementia and how our current servces can be adapted will be extremley important to providing an effective service. Scoping or mapping demand for an activity and education programme specifically to meet the needs of people living with dementia and their carers will be required.

Publicising the benefits of an activity programme and attracting people with dementia and their carers to the programme at a time most suitable to their needs for this type of support.

Attracting and applying for funding or renewing funding for this project and finding examples of best practice in this field. Partnership working is essential to our community work and therefore working with other partners to support the programme will be essential.
