1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
Through its members, networks and partnerships HWRCC can encourage communities to become dementia friendly and raise awareness of the part they play to support people living with dementia to live well in their community.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
We would need access to specialist information to enable us to cascade details and would look to forge a partnership between our organisations to deliver these outcomes.

Improve our organisation's understanding of dementia
We will arrange for all staff to attend/ have copies of dementia awareness sessions in order to have a greater understanding of dementia

Through our existing networtk we will promote the Dementia Alliance network and provide access to resources to enable them to become dementia friendly organisations.
