1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
The Community Pharmacy has a vital role in managing patients with dementia and supporting them their carers’ and families.
We aim to support the patients diagnosed with dementia or help identify potential patients to the GP.
We also support families and carers of patients with dementia on a daily basis as part of our professional role
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
The key challenge is ensuring that we pick up all potential patients and also help create a location or point where people can come for that support.
Staff training is a key issue as this will enable appropriate action and understanding for those users of our service.
2. Actions
-
Ensure all staff are provided with adequate training to meet the challenge
We now have two trained Demntia Champions in the organisation and all staff are now Dementia Friends. A Dementia Friends Champion has been appointed to deliver awareness sessions to colleagues and this is in progress
